Seeking a client friendly social worker to provide discharge planning for outgoing residents. Under the supervision of the Social Service Director, Discharge Coordinator will develop and implement discharge plans for current residents. Coordinator will also manage care coordination and communication of discharge plans to post facility providers and or family members.
